SOCIAL NEEDS ASSESSMENT RESIDENT HOUSEHOLD SURVEY

SOCIAL NEEDS ASSESSMENT RESIDENT HOUSEHOLD SURVEY

The Town of Smoky Lake is conducting an Assessment to better understand the social needs of residents in the Smoky Lake area including the County and Villages. Please have an adult in the household answer the questionnaire by considering the needs of all members of your household; it should take about 10 – 15 minutes to complete. Your completed questionnaire should be submitted by December 20, 2019.

Please submit your completed questionnaire by dropping it off at the Town of Smoky Lake Office – 56 Wheatland Ave. Smoky Lake, AB. If you wish to keep your responses confidential, envelopes are available. If you have any questions about this survey, please contact:

Marianne Prockiw-Zarusky (FCSS Program Manager) at 780-656-3674 or email: fcss@smokylake.ca

(Although the survey is being conducted by the Town of Smoky Lake the intent is to collect the responses of

everyone in the Smoky Lake County and we encourage everyone to participate.)

SOCIAL NEEDS ASSESSMENT RESIDENT HOUSEHOLD SURVEY

We are on TV this weekend!

Good morning Joan,

Know it’s a busy time with the fair just days away but I wanted to let you know we were in town this week and Smoky Lake will be featured on TV/radio and the web this weekend.

We were out to the Tree Improvement and Seed Centre, while there the manager Lee Charleson suggested we stop in at pumpkin park. We did and that got me thinking.

When we got back to the office I found some vintage footage in the archive vault from the fair in 1992. We’ve decide to put it in this week’s show.

You can Smoky Lake on CBC TV: Saturday at 10 a.m., Sunday at noon and Monday at 11 a.m. in a show called Our Edmonton.

After that we’ll post a link here. https://www.cbc.ca/player/news/Canada/Edmonton/Our%20Edmonton
